What Else Should I Know?

Our membership plan is not insurance and is not a qualified health plan under the Affordable Care Act.
Our membership plan includes a 30-day money-back guarantee. You can receive a full refund up to 30 days after purchase if no benefits of the plan have been used. Refunds after 30 days will be at the discretion of our practice.
Payment for any treatment(s) not included in the membership plan is due at the time of service.
The membership plan benefits may not be combined with insurance, other offers, or discounts.
Our membership plan does not cover procedures completed by specialists outside our practice.

Insurance

We are pleased to help you complete and submit your insurance forms as a service to you provided you supply us with all the necessary information we will require to help you. We will do our best to maximize your dental benefits. If you provide us with a current copy of your policy, we will also often help you interpret it.

A few things to know:

Our treatment proposals are based solely upon your personal dental needs and desires, and are not predetermined by the constraints of any insurance company.
We do not “treat your insurance plan”; we will treat you, and address your individual needs. The treatment plans we create will be co-developed with you, and with the best interests of your health in mind.
The amount of benefit will vary based on the parameters of your specific dental plan negotiated between your employer and the insurance company. Many PPO plans offer benefits even if you select a dentist that is not in-network.
Keep in mind that dental benefit plans will never pay for completion of your dental care. It is only meant to assist you and reduce your out of pocket costs.
Our office is not, and cannot be a part of the legal contract between you and your insurance company.

Please note that it is not always possible to diagnose and plan for every situation, especially with complex problems. We do our best to estimate as closely as possible, but unforeseen circumstances and situations can occur.
